Sorry there is a critical step missing in the FAQ. Now that you have your project created, click on it from your Recent Projects list. This will bring up the Project Portal. Scroll down the bottom and select Issue Resolution Service Administration. (That was the missing step.) This brings up the Form Manager. At the top is the option for punch list or Civil. Set to Civil. This will give you fields such as stationing, offsets, Pay items, etc. Below you will see Civil Default. Click on the pencil on the right side and you'll see the default form that you can modify to your organizational standards. Note the only field that is required is Subject, you can delete any other fields you don't want, add in fields, and change the Display Name (which is what the users on the mobile device will see.)
I will update the FAQ as there have been a few other minor changes plus this missing step and re-issue on the Communities shortly.
